Here are the forms to use for all applications to the Employment Relations Authority.

Each form is available as either an MS Word (version 6.0/95 document) or an Adobe Acrobat Portable Document Format (PDF) file. 

Whichever format you use, you will have to first print out the form and then complete it. Online forms for the Employment Relations Authority are not available.

Applicant lodging a new application

Undertaking in relation to application for interim reinstatement

Respondent providing a Statement in Reply

Lodging an application for investigation to be reopened

Lodging application for removal of a matter, or part of a matter, to the Employment Court
