We have provided these forms for use by employers responding to employees' written applications and notices. They are taken direct from from the Regulations made under the Parental Leave and Employment Protection Act. We provide two versions of each.

• Information that employers are required to give employees about paid parental leave under section 71T of the Parental Leave & Employment Protection Act 1987
• Form 2 – Notice to employee whose job is being kept open (PDF 13KB, MS Word 95/6.0 13KB)
• Form 3 – Notice to employee whose job is not being kept open (PDF 14KB, MS Word 95/6.0 13KB)
